Board Member Dashboard then Guests then Guest Waiver Settings

Waiver Enabled – Set this to Yes if your club will be collecting guest waivers. If not, set this to No as it will speed up some functions in the app.
Waiver Title – This title appears above the main waiver body text. Most likely it should include the club name followed by “Club Release and Waiver of Liability”.
Waiver Main Body – This is the main text for your waiver form. A copy of this text will also appear in the email sent to guests after completing their waiver (if you choose to enable emailing them).
Lock Main Body Text – Locking the main body text will prevent arbitrary edits of the waiver by future board members. Once your legal advisor has approved the waiver, we recommended locking it.
Addendum for Special Cases / Vendors – This text can be added below the main body for temporary addendums to the waiver. For example, if a 3rd party vendor such as an inflatable rental company asks for waivers or if a social event requires specific indemnification.
Addendum End Date – After this date the addendum will no longer appear.
In Perpetuity Text – This text appears below the main body and any addendums. The default text reads, “I agree that this waiver and release of liability is intended to be continuous, permanent, and in perpetuity. It shall remain in full force and effect forever, binding me, my family, my heirs, executors, administrators, personal representatives, and assigns, and shall apply to all future visits to or utilization of the club facilities.” We recommend keeping some similar version of that text, so guests (and staff for that matter) understand these waivers are kept in file and do not expire.
Require Primary Signer’s Phone Number – We recommend setting this to Yes. Requiring the primary signer’s phone number will help Front Desk Staff match an incoming guest to an existing waiver.
Require Member’s Name and Expected Date of Visit – Setting this to Yes will add three extra fields that must be completed – Member First Name, Last Name and Expected Date of Visit. Member Names are not publicly listed, guests must write in both first and last name. This feature is most useful for clubs that will be asking members to pre-register their guests (for example, if the member is hosting a pool birthday party and has a lot of guests coming). The expected visit date in particular can help staff plan for the number of guests attending and see who already has a waiver on file.
Ask for Minor Names – Unless your club is adults-only, set this to Yes.
Number of Minor Name Slots – Select the number of minor names fields that will be available for the primary guest adult to fill in.
Allow Two Adults from Household on Same Waiver – To expedite waiver signing, you can allow both spouses (or two adults from the same household) on the same waiver. If set to Yes the signature box will note both adults should sign or initial.
Add ‘I agree’ Checkbox – Require a checkbox under the waiver text to be toggled. If requiring a signature this option could be turned off as the action of signing implies the guest has read and is “making their mark” to agree.
‘I agree’ Text – This is the specific text next to the checkbox mentioned above. By default it reads “I have read the terms above and agree”
Require Signatures – We recommend setting this to Yes. If set to No, then the signature box will not appear on the waiver. If your settings allow the waiver to include 2 adults, the second adult can also sign or initial in the same signature box.
Show QR Code in Footer – This will display a QR Code at the footer of the page with the intention that guests can share the QR code with their spouses or other guests to help them sign too. If set to ‘Only show on devices logged in’, only Board Members can see the QR. The QR can be printed out or used in some other way at the Front Desk to help guests complete the waiver.
Text Header in Confirm Info section – Adjust the text that normally says ‘By proceeding, you confirm that the information below is accurate and that you are at least 18 years of age.’
Text Header in Additional Adult section – Adjust the text that normally says ‘If an additional adult from the same household is entering, fill in their info below….’
Text Header in Minor Names section – Adjust the text that normally says ‘Enter the names of all minors under age 18 for whom you are a parent, legal guardian, or otherwise authorized to sign…’
Text After Signing Waiver – Adjust the text on the ‘Waiver Complete’ page (what guests see after signing). The default text is ‘If requested, you can present this page to staff as proof of signing. To sign a new waiver or update your agreement with more family members, tap below… ‘
Text above QR Code – If you choose to keep the QR code on the Guest Form, you can adjust the text above it. The default text is ‘This QR code can be scanned by guests that need to complete our waiver…’
Email Signer(s) a Copy of Waiver – This will email a copy of the waiver to the adult signers. The email will contain the waiver title, body and any addendums and ‘in perpetuity’ text.
Email Copy of Waiver to Admin(s) or Archival Inbox – Comma separate any email addresses that should be CC-ed when a waiver is completed. This email will include the main waiver body text, addendum text and in-perpetuity text at the time of signing / completion. We recommend using a dedicated email address to function as an archive of all completed waivers.
